New Ontario Health Team of Northumberland website that can help improve system navigation

The Ontario Health Team of Northumberland is pleased to announce a new web site that features an integrated system-navigation platform designed to better support the community's patients, caregivers, residents and health-care providers.
The user-friendly website at https://www.ohtnorthumberland.ca was developed with a focus on the patient and caregiver experience, with tailored resources and accessible information aligned with the OHT-N's core-priority areas, including:
Access to primary and specialty care, including co-ordinating care pathways for those without access to a primary-care provider
Services for seniors and older adults with complex conditions, helping these individuals maintain independence, sustain their health and age well at home
Mental health and addiction support, with a focus on prevention, early intervention, continuous care and crisis response
The website also highlights the OHT-N's collaborative efforts in spearheading new initiatives to address identified community needs (including the expansion of Colborne's Rural Outreach Clinic in partnership with the Township of Cramahe).
Northumberland Hills Hospital President and Chief Executive Officer Susan Walsh (also OHT-N co-chair with Community Health Centres of Northumberland Executive Director Taryn Rennicks) said the goal is to create a connected, sustainable health-care system in which people can more easily find the care and support they need.
“We've heard from patients and caregivers that navigating services can be challenging,” Walsh stated.
“This new website was developed in direct response to these concerns, offering a trusted one-stop resource for local health services and supports.”
Suzanne Lawson, Chair of the Experience Partner Council, has also heard these tales of frustration.
“By bringing attention to our local health organizations, highlighting primary and acute-care services, and featuring where to access caregiver resources on one intuitive navigation platform, this redesigned website turns that maze into a single front door – saving time, reducing stress and letting people focus on their health instead of the search.”
Community members are encouraged to visit the site and provide feedback to help enhance the platform its usability. The website will be regularly updated to reflect changes in local healthcare services and public-health information.
